We realize how frustrating it is to juggle appointments, ensure medication refills, and follow complex health plans, especially if you are managing chronic conditions. Since treating chronic diseases can be overwhelming, MDChronic’s user-friendly platform and expert support makes your daily routine run smoothly, with less stress and more success.
How does MDChronic work?
Once you've enrolled with MDChronic, getting started is straightforward. First, we'll connect you with a dedicated health coach or physician to discuss your specific healthcare objectives and needs. If the physician sees the benefits of joining these programs, such as Remote Patient Monitoring and/or Chronic Care Management, and you agree to participate, we'll send you our free starter kit, which includes user-friendly smart monitoring devices, to help you start tracking vital signs and other important health metrics right away.
Next, you'll have access to our virtual care platform and mobile app, where you can schedule online consultations, log health data, and receive personalized insights. Your health coach will walk you through each stage, helping you modify your care plan, handle medication refills, and answer any concerns you have about managing your chronic disease.
What chronic conditions are managed with MDChronic?
Numerous chronic illnesses, such as diabetes, high blood pressure, cardiovascular issues, and more, are supported by MDChronic. Our virtual care platform adjusts to your lifestyle, enabling you to conveniently check vital signs, arrange online consultations, and receive continuous coaching from our committed health professionals, regardless of how many ailments you're treating.
Patients all throughout the country rely on MDChronic because of its all-encompassing, stress-free approach to chronic care. We enable you to manage your health from any location, regardless of whether you live alone, have limited mobility, or just wish to avoid frequent clinic visits.
MDChronic offers you the adaptability and individualized support you require to succeed, regardless of the scope or complexity of your healthcare requirements.
How does insurance coverage work with MDChronic?
If you have Medicare or a Medicare Advantage plan (with Part B), you can generally expect 80% of the program cost to be covered. A secondary insurance plan may cover the rest of 20%.
Currently, MDChronic does not cover commercial insurance plans unless you’re comfortable covering those expenses out-of-pocket. If you have questions about specific costs or coverage details, our support team is here to help.

Do family members or relatives qualify for MDChronic?
Only the member enrolled is eligible or qualified to receive MDChronic’s online healthcare services. To find out if any benefits for family members or relatives are currently being provided, you can enquire with our support team.
How can I talk to my health coach?
Video chats, phone calls, and in-app messaging are just a few of the ways you may interact with your health coach. You can contact your coach whenever it's most convenient during your health journey.
How do I get started with a prevention or hypertension program?
Getting started with a care program is simple. You can sign up through the MDChronic website, where you’ll complete an initial health assessment. From there, you’ll connect with a health coach who will work with you to develop a personalized plan for managing your health condition. Whether it’s prevention or hypertension, your coach will guide you through the recovery process.
Can any member receive ongoing support from a health coach?
Every member enrolled in a healthcare program with MDChronic has access to receive ongoing support from a health coach. Whether you're working on managing a chronic condition, improving your mental health, or making lifestyle changes, your coach will be with you every step of the way.

We take your privacy seriously. All your personal and health data is protected by strict security protocols, including encryption and HIPAA compliance. Your information is only accessible to your health coach and authorized team members, ensuring confidentiality at every step.

What type of provider interaction sessions is available?
MDChronic offers both one-on-one coaching and group interactions.
